does this catch can come with a mounting bracket?

Hi Brandon, it does not include a bracket.
I assume the cap at the top of the can is removable to install the breather filter, but what are the extra fittings for that I see in the pictures?

Hey Alfred, the two smaller fittings can replace the main port fittings in case you need 9mm instead of 15mm. The one larger fitting can replace the cap on the top if you want to run the breather filter.
